Community Health Systems Catalog

The Community Health Systems Catalog is an innovative and interactive reference tool on country community health systems. Search a wide range of information about community health systems across 25 countries, learn more about national policies and programs, view snapshots of family planning activities by country, and analyze the data on the different cadres within each program and country.

APC's grantee World Vision implements the TEAM Colombia project with local partners Fundación Saldarriaga Concha, Fundación Cirec, and Instituto de Ortopedia Infantil Roosevelt, which enables people with disabilities (PWD), especially women and girls, to attain and maintain maximum independence to fully participate in all aspects of life in Colombia. Through TEAM project initiatives, more than 1,900 PWD will be reached with services to help them gain maximum independence in their communities.
